Calcium carbonate is an inorganic basic salt that neutralizes the hydrochloric acid in gastric secretions. It also inhibits pepsin action by increasing pH and adsorption. Increases in bicarbonate ion (HCO3-) and prostaglandins may have cytoprotective effects. The neutralization of hydrochloric acid produces calcium chloride, carbon dioxide, and water. 90% of calcium chloride is converted into insoluble calcium salts (e.g. calcium carbonate and calcium phosphate).
Sodium bicarbonate is a systemic alkalizer that raises blood pH, raises plasma bicarbonate, and buffers excess hydrogen ion concentrations, reversing the clinical manifestations of acidosis.
It is also a urinary alkalizer, increasing the excretion of free bicarbonate ions in the urine and thus raising urinary pH. The actual dissolution of uric acid stones may be accomplished by maintaining alkaline urine. Sodium bicarbonate acts as an antacid, chemically neutralizing or buffering existing amounts of stomach acid but has no direct effect on its output. This action raises the pH of the stomach contents, providing relief from hyperacidity symptoms.
The product's mode of action is physical and does not rely on systemic circulation absorption. After consumption, the product swiftly combines with stomach acid to produce a raft of alginic acid gel with a pH that is close to neutral and floats on the stomach contents, successfully preventing gastro-esophageal reflux for up to 4 hours. In extreme circumstances, the raft itself may reflux into the esophagus instead of the contents of the stomach and have a demulcent effect.
In particular, tetracyclines, digoxin, fluoroquinolone, iron salt, ketoconazole, neuroleptics, thyroid hormones, penicillamine, beta-blockers (atenolol, metoprolol, propranolol), glucocorticoids, chloroquine, and biphosphonates (diphosphonates) and estramustine should be administered two hours apart before taking this medication.
Patients with known or suspected hypersensitivity to the active ingredients or to any of the excipients should not use this medication.
